GetADoctorsNote.com, the Site, and products offered herein are not a substitute for medical diagnosis, care, or treatment of a physician, psychologist or other health professional. You should always seek and rely exclusively on the advice of your physician, psychologist, or other health professional.
  2. The Site and any applications are not intended to create any physician-patient relationship, and your use of the Site does not, and will not, create a physician-patient relationship between you and GetADoctorsNote.com.

This provision shall survive termination.

You agree that use of the Site shall be deemed to have occurred in the State of Nevada.  As such, any claim or dispute relating to GetADoctorsNote.com, the Site, or your use or reliance on the Site, shall be construed in accordance with the laws of the State of Nevada without regard to any conflict of laws provision. You and GetADoctorsNote.com agree to be bound and shall be subject to the exclusive jurisdiction of the local, state, or federal courts located in Clark County, Nevada.

BY USING GETADOCTORSNOTE.COM OR ACCESSING THE WEBSITE OR APPLICATIONS, YOU ACKNOWLEDGE AND ACCEPT THAT SUBMITTING YOUR TELEPHONE NUMBER OR VOIP INFORMATION TO GETADOCTORSNOTE.COM CONSTITUTES AN INQUIRY TO GETADOCTORSNOTE.COM, AND THAT GETADOCTORSNOTE.COM MAY CONTACT YOU AT THE NUMBER SUBMITTED EVEN IF SUCH NUMBER APPEARS ON ANY STATE OR FEDERAL DO NOT CALL LISTS (TAKING INTO ACCOUNT INQUIRY EXCEPTION TIME FRAMES AS APPROPRIATE).

If any provision of the foregoing is held to be invalid or unenforceable, such provision(s) shall be fully severable, and in place of such provisions(s) shall be substituted a similar provision(s), which shall be legal, valid and enforceable. Any failure to act with respect to a breach by you or others does not waive our right to act with respect to that breach or subsequent or similar breaches of this Agreement. This Agreement sets forth the entire understanding and agreement between us with respect to the subject matter therein.
